- Visit the location and complete the Blue ExpressPayment Form.
- Enter "4963" under "Receive Code".
- Enter "Southwest Bank " under "Company I Am Paying", and "Fort Worth, TX " as the address.
- Enter your bank account number under "Account Number".
- Enter the amount of cash you want to deposit under "Payment Amount".
- Provide the agent with your completed ExpressPayment form and cash for your deposit amount and the Express Payment fee.
- Please keep the reference number that confirms your transaction.
